Procedures for Departments Requesting Access to STAR

Instructions for Requesting a New Account

1. Obtain a 'Banner / STAR Account Request' form at: STAR ACCOUNT (PDF Format)
2. Complete the 'Banner / STAR Account Request' form and obtain the appropriate signatures.
3. Sign the 'Code of Responsibility' form found at: CONFIDENTIALITY (PDF Format)
4. A memo(s) must accompany the request form justifying why the access is needed and what type of access is required (i.e. user works with student addresses, grades, schedules, billing, financial aid, etc.). Address the memo to each STAR Module Chair (located below) for the access required.
5. Send the 'Banner / STAR Account Request' form, the 'Code of Responsibility' form, and memo(s) to the Admissions and Records Module Chair.

SSM (Students Systems Management) will notify the requestor when the access is completed and when the next STAR training is available.

Instructions for Modifying an Existing Account

1. Obtain a 'Banner / STAR Account Request' form at: STAR ACCOUNT (PDF Format)
2. Complete the 'Banner / STAR Account Request' form checking 'Modify Account' block and obtain the appropriate signatures.
3. If additional access is being requested, a memo addressed to the Module Chair must be submitted justifying why the access is needed and what type of access is required (i.e. user works with student addresses, grades, schedules, billing, financial aid, etc.).
4. Explain the modification in the 'Comments' section of the form.
5. Send the 'Banner / STAR Account Request' form, the 'Code of Responsibility' form (if necessary), and memo(s) (if applicable) to the Admissions and Records Module Chair.

Instructions for Deleting an Existing Account

1. Obtain a 'Banner / STAR Account Request' form at: STAR ACCOUNT (PDF Format)
2. Complete the 'Banner / STAR Account Request' form checking the 'Delete Account' block and obtain the appropriate signatures.
3. Explain the reason for deletion in the 'Comments' section indicating the STAR account to delete.
4. Send the 'Banner / STAR Account Request' form to the Admissions and Records Module Chair.
